    What’s required for claim?

    Home Insurance
    • Claim Intimation(by letter or email)
    • Duly completed claim form.
    • Details of loss estimate from a recognized/authorized consultant/dealer/firm/repairer towards repair/replacement.
    • FIR lodged on the concerned Police authority.
    • Original Report from the concerned authorities of Police, Municipal Authority, VDC etc.  
    • Spot Investigation Report (SarjaminMuchulka) of the loss.
    • Report from hydrological and meteorological department.(In case of loss due to storm, tempest, hurricane, cyclone, heavy rainfall, flood)
    • Report of your Technician/expert certifying the cause and extent of damage/loss where the same can be repaired or needs replacement.
    • Fire Brigade report (if any).
    • Tax invoice of repair/replacement.
    • Invoice of damaged property showing actual value at the time of purchase. (Tax invoice, Commercial Invoice, Custom Documents etc.)
    • Audit Report/Documents showing the book value of damaged item.
    • Details of stock as on date of loss supported by Stock book, Purchase bill, Sales bills.
    • Any additional documents and information that may be required during the investigation by Insurer or Adjuster.
    Motorcycle/Scooter Insurance (2 wheeler)

    In case of Comprehensive (Full) coverage:

    • Claim Intimation letter(by letter or email)
    • Estimate/Quotation from the authorized dealer/repairer.
    • Copy of Driving Licence & updated Vehicle Registration Book (Blue Book).
    • Original Report from the concerned Police authority.
    • Original VAT Bills/Cash Memos towards repair/replacement.

    In case of Thrid Party coverage/ / Rider and Pillion Rider:

    • Third Party Vehicle/Motorcycle
      • Estimate/Quotation from the authorized dealer/repairer.
      • Copy of Driving Licence, Motorcycle Registration Book (Blue Book)
      • Police Report.
      • Original VAT Bills / Cash Memos towards repair / replacement.
      • Third Party’s Vehicle/MotorcyclePolicy copy
    • Injury /Death
      • Police Report.
      • Emergency Card issued by the treating Hospital/ Nursing home
      • Attending Doctor’s/Hospital’s prescriptions/Nurse’s Daily Record/ Doctor’s prescription including chit issued by the Nurse on duty.
      • Discharge Summary in original.
      • Original bills incurred for medical expenses.
      • Agreement (Milapatra) in original done in presence of the concerned legal authority.
      • Death Case:
    • Post Mortem Report
    • Certificate of relationship
    Personal Accident

    In case of Injury

    1. Claim intimation (by letter or email)
    2. Claim Form duly completed by the claimant with stamp
    3. Medical Reports duly completed by the concerned doctor/hospital with stamp
    4. Attending doctors/hospital’s Prescriptions/reports and Discharge Summary if admitted in original or copy.
    5. Original Cash bills (Tax invoice) incurred for treatment expenses.
    6. Your Confirmation about date of joining office by the injured staff after leave.
    7. Certified copy of attendance register/Leave details
    8. Monthly salary of injured staff.

    In case of Accidental Death

    1. Claim intimation (by letter or email)
    2. Claim Form to be filled in by concerned office with official stamp/seal
    3. Attending doctors/hospital’s prescriptions/reports in original or copy.
    4. FIR Lodged on the concerned Police Authority.
    5. Original Police Report.
    6. Spot investigation report (SarjaminMuchulka.)
    7. Post Mortem Report.
    8. Death Registration Certificate.
    9. Dead Body hand over certificate (Las SadagatPurji.)
    10. Certificate of relationship.
    11. Other relevant documents, if any.
    Private Vehicle Insurance (4 Wheeler)

    In case of Comprehensive (Full) coverage:

    • Claim Intimation letter(by letter or email)
    • Estimate/Quotation from the authorized dealer/repairer.
    • Copy of Driving Licence & updated Vehicle Registration Book (Blue Book).
    • Original Report from the concerned Police authority.
    • Original VAT Bills/Cash Memos towards repair/replacement.
    • Duly Completed Claim Form

    In case of Thrid Party coverage

    • Third Party Vehicle/Motorcycle
      • Estimate/Quotation from the authorized dealer/repairer.
      • Copy of Driving Licence, Vehicle Registration Book (Blue Book) & Route permit (if applicable).
      • Police Report.
      • Original VAT Bills/Cash Memos towards repair / replacement.
      • Third Party’s Vehicle/Motorcycle Policy copy
      • Duly completed Claim Form
    • Injury /Death
      • Police Report.
      • Emergency Card issued by the treating Hospital/ Nursing home
      • Attending Doctor’s/Hospital’s prescriptions/Nurse’s Daily Record/ Doctor’s prescription including chit issued by the Nurse on duty.
      • Discharge Summary in original.

    Travel Insurance

    In case of Non-Emergency

    • As soon as insured return their home country, they must contact the Insurance Company who has issued policy to them within 31 days of their arrival.

    In case of Emergency Medical Expenses

    1. The Insured must contact to the TPA immediately in the event of emergency
    1. Claim form to be completed by claimant
    2. Original cash invoice/bill incurred for the treatment
    3. Doctor / Hospital prescriptions and reports
    4. Travel itinerary / E-tickets
    5. Copy of passport with departure and arrival stamped pages
    6. Original insurance policy

    In case of Loss of Checked Baggage

    1. Claim form to be completed by claimant
    2. Property Irregularity Report (To be obtained from the airline immediately)
    3. Confirmation from the airline regarding details of loss.
    4. Travel itinerary (E-tickets)
    5. Receipts for items
    6. Copy of passport with departure and arrival stamped pages
    7. Original insurance policy

    In case of Delay of Checked Baggage

    1. Claim form to be completed by claimant
    2. Property Irregularity Report or Non delivery certificate for baggage (To be obtained from the airline immediately)
    3. Receipts for the emergency items purchased
    4. Confirmation from the carrier (airline) regarding the length of delay or baggage delivered voucher and reasons for the delay
    5. Travel itinerary (E-tickets)
    6. Copy of passport with departure and arrival stamped pages
    7. Original insurance policy

    In case of Travel Delay

    1. Claim form to be completed by claimant
    2. Statement to be obtained from the carrier confirming the length of the period and exact nature of the delay.
    3. Boarding pass
    4. Travel itinerary (E-tickets)
    5. Copy of passport with departure and arrival stamped pages
    6. Original insurance policy
